The Max and Arlyne Hurwitz House, 1966 Mid Century Modern in Madison, Wisconsin

This is a very unusual and unique house. I highly suggest checking out the virtual tour to see how the floor plan twists and turns. It once had an indoor swimming pool, which would be my favorite thing if it were still there. It's not specifically stated in the description or any other bits I found in my research, but the virtual tour does appear to show the systems for the pool, so maybe it's just covered and not completely filled in, which might be why the ladder is in the floor surrounded by carpet.
